If you have an upcoming Mental Health and Substance Abuse Screening appointment scheduled at WCMHC, it is recommended that you complete the appropriate paperwork prior to your appointment and bring it with you to the office. This will allow the screener you will be seeing to review the information beforehand and spend more time discussing your current concerns and treatment options during the appointment.
New Client forms can now be completed using Adobe Acrobat Reader and submitted via email. Please download the applicable forms below and click here for instructions on how to sign electronically. Once finished, please email the completed documents to forms@wcmhc.com.
