What is Case Management?
Case Management at Winchester Community Mental Health Center is a supportive service that helps you navigate life’s challenges, whether it’s getting to appointments, understanding paperwork, or finding the right resources. Case Management is designed to work at your pace. You don’t have to figure everything out alone. We are here to help make things easier.
What is a Case Manager?
Your Case Manager is someone who works with you to make sure you are getting the help you need. They don’t replace your therapist or doctor…..they partner with them to make sure everything in your care plan fits together. Think of a case manager as your go-to person when:
- You need help scheduling or remembering appointments.
- You’re not sure what services are available.
- You’re overwhelmed by forms or paperwork.
- You want someone to help advocate for your needs.
- You’re trying to stay on track with treatment and life goals.
What Happens When you Start Case Management?
When you begin Case Management, your Case Manager follows a simple, supportive process to get you connected to the help you need. The Case Manager starts by getting to know you. Your strengths, challenges, and what you’d like help with. The first step helps to understand your needs and goals. Next, the case manager works with you to build a personalized Case Management treatment plan. This plan outlines the steps you and your Case Manager will take together to address your needs and connect you to the right resources. Once your plan is in place, your Case Manager begins linking you to services like counseling, psychiatry, housing programs, transportation help, or school support. The Case Manager will make referrals, help with paperwork and coordinate care between providers. As you move forward, the Case Manager will check in with you regularly to see how things are going, adjust your plan if needed, and make sure you are getting the support that works best for you!
Examples of How Case Management Can Help You:
Your Case Manager can help you get connected to the services and support you need to feel more stable, healthy, and supported. Here are some of the things they can help with:
- Health & Wellness: Programs such as therapy/counseling, psychiatry, substance use treatment, specialty medical care, dentistry, primary care, nutrition.
- Basic Needs: Food, clothing, hygiene supplies, housing or shelter help, utility or rent assistance, domestic violence support.
- Transportation Help: Rides to appointments, bus passes or schedules, help with mileage reimbursement.
- Paperwork & Benefits: Medicaid, SNAP (food stamps), WIC, social security or disability forms, help with ID, birth certificates, etc.
- Education & Family: School support and IEP/504 help, parenting programs, childcare, Head Start, referrals, CPS/APS, or foster care support.
- Jobs & Money: Job training or applications, budgeting help, unemployment or workforce help.
- Advocacy & Support: Talking with doctors, schools, or service providers and finding local support groups.
Who is Eligible for Case Management?
West Virginia Medicaid (MCO):
- You have WV Medicaid Insurance and are enrolled with a WV Medicaid MCO plan such as Wellpoint, Aetna Better Health of WV, or The Health Plan;
- Have a mental health, emotional or substance use diagnosis;
- Need help managing daily tasks like appointments, transportation or connecting to services.
Virginia Medicaid (MCO):
- You have Virginia Medicaid and are enrolled with a MCO such as Aetna Better Health of VA, Optima, Anthem Healthkeepers Plus, Molina, or United Healthcare;
- Have a primary diagnosis of a Substance Use Disorder (SUD)
